Throughout Costa Rica, health-care workers only used soap, water and towels to clean their hands when providing care. Water shortages are an issue during the dry season, so there was a need for an alternative method for cleaning hands in health care.

It was also important to comply with WHO recommended actions to prevent HAI, including from drug-resistant bacteria. Early reports showed hand hygiene compliance was as low as 40%.
