What type of health-care facility or organization are you?
Ain Shams University Specialized Hospital is a 1000 bed university hospital situated in Cairo, Egypt.
What was the situation at your facility with respect to hand hygiene and HAI before you initiated the hand hygiene initiatives?
Our infection control system has been in place since 1989, hand hygiene represents an integral part of the programme.- Refresher training courses are regularly held every year, especially for nurses. We had a hand hygiene campaign in 2008 as a part of the "IFIC SIG-Hand Hygiene " activity. Since then we have shifted to hand hygiene using alcohol-based products.
What were the goals of your initiative?
The main target was to increase the compliance of health-care staff with hand hygiene, in particular after we shifted to the use of alcohol-based products. We targeted the initiative to minimize transmission of infection especially in ICU departments.
What actions have you planned or already put in place to improve hand hygiene at your facility?
A hand hygiene campaign was done over one month (April 2009) in which we designed a pocket bottle with alcohol gel and an observation sheet was formulated focussing on the 5 moments. This was started after an orientation lecture in which we explained the WHO recommendations for hand hygiene.
Did you encounter any particular problems while planning and implementing your strategy?
Of course the main problem was the supply of the alcohol-based product, but we could overcome this by having the alcohol gel as donation from one of the companies in Cairo, who consider much to share in an activity through our organization .
What were the outcomes of your initiatives?
We organized properly the campaign and the follow up of the implementation of the 5 moments using the method of pocket bottle in all the intensive care units in our hospital using an observation sheet on daily basis for one month.
What were your key learning points?
Yes, in one of the ICUs we will revise the relatively poor compliance using the method of pocket bottle. However the overall the compliance of hand hygiene was excellent and the implementation of the 5 moments was perfect specially for moment 3. Implementation of moment 2 needs to be reinforced as well.
What are your future directions?
Continuation of implementation of the 5 moments will remain our objective, we will try to keep the supply with alcohol-based products. Follow up of infection transmission is done by the ongoing surveillance and notification of infected cases. Watching the occurence of outbreaks is cosidered as well.
We celebrated the event of "World day for Hand Hygiene" and we promoted for the new slogan: "Save Lives and Clean Your Hands" in our hospital, ASUSH, under the patronage of the society of SPIC-EGYPT. Results of our campaign were presented as example of implementation of the 5 moments of hand hygiene using the method of pocket bottle.
