This article describes the development of health impact assessment in Thailand, viewed as "taking place in difficult circumstances but with high hopes". The paper describes how the reform of the national health system was conducted alongside the drafting process of the National Health Act, and describes why Thailand chose to develop HIA through the development of healthy public policy, rather than as an additional component to environmental impact assessments. The challenges facing the Thai HIA community are discussed and recent experiences are shared.
