Retrospective record review
This method estimates incidence of harmful incidents and counts harmful incidents during the whole duration of the hospital stay. The data is obtained from medical records from past hospital admissions. This method can be used either to count harmful incidents and identify their causes, or to monitor and improve patient safety achievements (although the information gathered will not reflect the current situation).
