Mali
The primary HRH challenge in Mali is not health workers in absolute numbers, but maldistribution of health workers throughout the country, particularly a lack of health wokers in poor, remote areas far from the capital. Approximately 31% of physicians, and 53% of allied health workers worked outside of the capital, and over 50% of private health facilities were located in the capital (Strategic Human Resources for Health Development Plan (PNS/RSS), 2009-2015). There is also low motivation among public health care workers due to insufficient financial and resource incentives, resulting in a loss of the most qualified health workers to the private sector, particularly specialists and technical staff. This is compounded by a lack of recruitment from private training institutions, lack of adaptation of curricula to the needs of employment, and inadequate planning for specialized staff. In addition, there is inadequate coordination of HRH management and planning of training needs; over-centralization of personnel management actions; and a lack of quality staff training due to the absence of educational regulations and lack of financial and physical resources at training institutions (PSN/RSS, 2009-2015).
In an effort to overcome these challenges, Mali has created both a National Policy for the Development for HRH and a National Strategic HRH Plan to operationalize the HRH policy in 2009. The National Policy and National Strategic HRH Plan cover the following strategic areas: organization of the function of HRH; HRH capacity building (initial and continuing); management recruitment and placement; career management; and health worker motivation. Other achievements include: creation of a national Directorate of Human Resources for Health; creation of a national HRH observatory (L'Observatoire National des Ressources Humaines pour le secteur de la Santé, du Développement social et de la promotion de la Femme); creation of an HRH working group; and full participation in the CCF process with assistance from the Alliance.
COUNTRY COORDINATION AND FACILITATION (CCF) IN MALI:
Mali has completed Phases 1-3 of the CCF process: creating an HRH coordination mechanism; conducting an HRH situation analysis; and creating a fully costed HRH strategic plan. Mali is currently in Phases 4 and 5. The country has begun implementing and monitoring its strategic plan and has been successful in mobilizing partner resources for HRH interventions.
