Health impact assessment

    Overview

    Health impact assessments (HIA) provide decision-makers with sound information about the health implications of policies, programmes and projects, ranging, for example, from construction of a waste incinerator to the redesign of an urban neighbourhood.

    WHO response

    WHO/Europe provides support to Member States on HIA by developing methodologies and tools, carrying out assessments and reviews, and advising on policy options.

    Commonly agreed upon tools for carrying out HIAs are not yet available, although provisions for HIA in policy-making are included in international legislation (e.g. European Union directives), legally binding agreements (e.g. the Strategic Environmental Assessment or SEA Protocol), and policy commitments (such as those of the ministerial conferences on environment and health).

    As mandated by ministerial conference declarations, WHO/Europe works to fill this gap by developing HIA methodologies and tools for use by national or local authorities or institutions. Such methodologies and tools are usually piloted through projects.

    WHO/Europe also promotes the integration of health in environmental assessments through building capacity at local, national and subregional level and supporting Member States in the implementation of the SEA Protocol.

    Integrated HIAs focus on how policies, plans or projects affect health through their environmental, economic and social impacts. Examples include integrating health issues and policies in environmental performance reviews and making recommendations for progress in environmental health.

    Overview

    WHO/Europe supports the integration of health into environmental assessments, such as environmental impact assessments (EIA) and strategic environmental assessments (SEA), and contributes to implementation of the SEA Protocol with regard to inclusion of health considerations.

    This is done through capacity building as well as through developing methods and tools for the HIA of specific sectors within projects.

    EIA should consider, among other issues, the effects of a project on health, but this aspect is often assessed poorly or not at all.

    SEA facilitates the identification and prevention of possible environmental effects right from the start of a project, and enables environmental objectives to be considered on a par with socioeconomic ones. SEA, undertaken much earlier in the decision-making process than EIA, is therefore a key tool to prevent ill health and tackle health inequalities. Fields of application are diverse, transport being the most extensively explored.

    Including health in environmental assessments

    Policy decisions made outside of the health sector influence many determinants of health. However, the potential health impact of such policies is often not, or only poorly, assessed. To protect health, WHO/Europe’s Health 2020 policy framework aims hence to support governments in fostering intersectoral action.

    In line with Health 2020 and the declarations of ministerial conferences on environment and health, WHO/Europe 

    • advocates for the integration of health in environmental assessments and policies through the use of health impact assessments;
    • builds national capacity on health impact assessment;
    • develops methods and tools for health impact assessment.

    The fields of application are very diverse, with transport being the most extensively explored.
