Are you between 12 and 19 years? This survey has been developed to get your views on health. We want to know what health issues are affecting you, and what health services you need. What influences your health decisions and behaviours?

Survey responses will be collected, analysed, and help to shape the report. The survey is confidential and all answers will be anonymous.
If you are under 18, you must have your parent or legal guardian’s consent to take part in the survey.
The survey opens 15 July 2013 and will be closed on 1 October 2013.
