Background

Following the adoption of ICD-11 by the Seventy-second World Health Assembly in May 2019 WHO HQ initiated ICD-11 implementation preparations at country level in close collaboration with WHO regional and country offices. Development and piloting of the ICD-11 Application Programming Interface (API) integration in software applications and health information systems used for morbidity and mortality data collection is one of the key components of the ICD-11 implementation preparations.

In 2020, Ministry of Health of Uganda developed an application on DHIS2 (District Health Information Software version2) that can bring the concept of ICD-11 (diagnostic, code search, etc) to the DHIS2 platform serving the data collection of facility mortality reports, using the WHO recommended international form of medical certification of causes of death (MCCOD). With the increasing interest from countries to use ICD-11, the WHO Data and Analytics (DNA) department in the Division of Data Analytics and Delivery for Impact (DDI) contracted the Health Information Systems Program (HISP) at the University of Oslo in Viet Nam to develop more generic standard tools and mechanisms to support country implementation, especially amongst those that already use DHIS2 as the national health management information system (HMIS) platform. This is work was carried out with the technical contribution of both WHO and University of Oslo/HISP who develops and maintains the core structure of the global DHIS2/WHO collaboration.

This initiative resulted in a prototype as proof of concept of a DHIS2 Application for ICD-11 and causes of deaths (DHIS2 CoD App). This tool has potential as a global solution that would enable not only the use of WHO MCCD and ICD-11 for coding causes of deaths but also to run and export WHO Analysing Mortality Level and Cause-of-Death (ANACoD3) for further analysis, while hosting a built-in analytical dashboard for standard analysis.

The finalization of this prototype into a DHIS2 core App that meets DHIS2 requirements will have an impact on the 60+ countries that currently use DHIS2. The final DHIS2 CoD ICD-11 core App will be a “plug-in tool” fitting the national DHIS2 structure as part of an integrated health information system in general, as well as an integral part of the mortality surveillance and reporting system in particular. It will promote and enable facilities and sub-national health managers to access, analyse and use the data on CoD for mortality and burden of disease analysis.

Requirements - Work to be performed 

The vendor will work with the WHO technical team at WHO headquarters as well as selected countries for testing and their respective regional offices to finalize the development of the DHIS2 CoD ICD-11 App as a core application in the DHIS2 platform.

Objective 1: DHIS2 CoD ICD-11 App is made available and ready to be tested in selected countries.

Output 1.1: DHIS2 CoD ICD-11 App will include the following features:

  • WHO electronic MCCD form with embedded digital mortality rule base; rule engine for automated selection of Underlying Cause of Death.
  • Capacity to generate DHIS2 ID specification.
  • ICD grouping for underlying cause of death: App will group ICD-11 codes based on WHO HQ recommendation of ICD-11 grouping.
  • Generation of customized death certificate.
  • ANACoD3 realtime export.
  • Standard and custom dashboards based on multiple ICD-11 special mortality tabulation lists and WHO standard analytics.
  • Translation support on user interface texts.


Objective 2:  DHIS2 CoD ICD-11 App is tested and functional in testing countries:

Output 2.1: All the modules including latest three version of DHIS2 software are tested in countries.

Output 2.2: Country support to implement the App as part of the national DHIS2 HMIS platform.

Objective 3: DHIS2 CoD ICD-11 App is available on the DHIS2 Apps Hub.

Output 3.1: Documentation and user manual in English: detailed documentation about the App including admin user manual for set up and end user manual for use of the App.

Output 3.2: App is available for download on the DHIS2 Apps Hub, and compatible for the latest 3 versions of DHIS2

Characteristics of the Provider

  • Extensive knowledge and experience in the development of core function development of DHIS2 as software and experience in country implementation and support is highly desired.
  • Extensive experience in country implementation, especially on mortality and morbidity using different DHIS2 functionalities and WHO mortality codes.
  • Extensive experience and in-depth knowledge of the DHIS2 WHO facility data standard packages and their implementation.
  • Extensive knowledge and experience in the development of application and extension on DHIS2 platform as well as integration with other core functions in DHIS2 to support integrated HMIS in countries.
  • Experience in the development of training materials, delivery of training, coaching and IT support during implementation.
  • Member of the DHIS2 community and core developers is an asset.

Place of assignment 

Provider’s office, travel is not expected as part of the assignment.
