On 25 July 2025, the WHO Patient Safety and
Quality of Care Unit hosted a global webinar on Patient Safety Incident
Reporting and Learning Systems (PSIRLS), as part of the Global Patient Safety
Webinar Series. The session brought together stakeholders from across health
systems to discuss key strategies for strengthening PSIRLS, including policy
options for implementation, essential components for creating an enabling
environment, and practical approaches to incident investigation and learning.
Participants heard experiences from countries such as South
Africa, Spain, India, the United Kingdom, Italy, and Singapore. Discussions
focused on turning incident data into actionable insights, tailoring systems to
local contexts, and applying frameworks such as sentinel event reporting and
national safety investigation models. The session also featured the latest WHO
guidance on PSIRLS and highlighted key implementation challenges faced
globally.
