Overview

The HHFA is a comprehensive health facility survey that assesses the availability of health facility services and the capacities of facilities to deliver services at required standards of quality. HHFA questionnaires are based on the four HHFA modules: service availability, service readiness, quality of care, and management and finance. The questionnaires are available in “stand-alone” and “combined” formats. Each of the four HHFA modules contains a set of stand-alone questionnaires that may be “Core”, “Core+Additional” and/or “Supplementary” questionnaires.

  • A Core questionnaire contains only core questions.
  • A Core+Additional questionnaire contains both core and additional questions.
  • A Supplementary questionnaire does not include questions from the other HHFA questionnaires and may collect data through a different methodology.

Questionnaires for the service availability, service readiness, and management and finance modules collect data through a facility audit methodology that involves interviews with health care providers and observation of items and conditions within facilities. The questionnaire for the quality of care module collects data through a record review methodology that involves assessment of a sample of individual patient records.
